During the Summer term, parents will be asked to complete the following forms:
- GDST Student Health Form
- Allergies and Intolerance Form
- Ethnicity and Nationality Form
- Parental Permissions Form
- New Pupil Additional Contacts
- Junior ICT Acceptable Use Agreement
Please complete these forms as directed through your admissions portal account.

All forms must be completed online and submitted before your daughter can be admitted to WHS in September.
